The following items are needed for registration:

Birth Certificate

Social Security Card

Immunization Records

3 Proofs of Address
(examples: current utility bill, rental agreement, purchase contract)

**8 th Grade Report Card

High School Transcript

*8 th Grade LEAP Scores

**For incoming freshmen only:   Report card must state whether student has been promoted or retained for the 07-08                                                       school year.
